After completing the form, candidates are requested to submit it along with other relevant documents and proof of original deposit to SR-01 at SR-10: OTS, Office No. 01, Crescent. Shopping Mall, Central Avenue Road, Phase 6, Bahria Town, Islamabad.
A softening of up to 5 years can be provided in the upper age.
